card you are required to provide the following documents: ·

of Child’s long form document (on legal size paper) ·

When renewing your Status
Card: ·
In
Person: it is required that you have
two pieces of Identification. One
preferably a birth certificate and a health card, driver’s license, etc. NEW Photos no

Treaty Payment Applications
are available through our membership office.
You may also apply through Indian and Northern Affairs Canada. You may also get an
application by clicking here. GAS CARD APPLICATiON
In order to apply for a Gas
Card you must be 16 years of age and over.
Applications are available through this office or you can PRINT ONE HERE. You will need Acrobat Reader for this file, if you do not have it you may download it for free
from HERE. A written request from the
applicant must be mailed or faxed directly to the Ministry of Revenue along
with a legible copy of your valid status card (front and back). Please include your
telephone number, mailing address and reason for requesting the card. Applicants who have not
received their gas card within three weeks should contact the Ministry of
Revenue. marriage, divorce and
DEATH CERTIFICATES
In order for us to update
our records: ·
All
Band Members are required to register their marriage. Please send a photocopy of your marriage
certificate. ·
Should
your marriage end in divorce, please send a photocopy of your divorce papers. ·
If
and when you wish to report a death, a copy of the Death Certificate is
required. Please Note: if you do not report your marriage or
